Fees & Payment Information
Clinical Evaluations
Clinical evaluations, including neuropsychological and diagnostic assessments, typically begin with an initial intake appointment. During this appointment, we will review the referral questions, relevant background information, and determine whether a full evaluation is indicated.
An intake fee is collected at the time of scheduling. If a full evaluation is recommended following the intake, a testing deposit will then be collected. The testing deposit will be adjusted to account for the intake fee already paid. If a full evaluation is not recommended, no additional deposit will be required.
Regarding Insurance
I do not participate with insurance panels and do not provide documentation for insurance reimbursement (such as superbills). This model allows me to offer comprehensive, individualized evaluations without the restrictions imposed by insurance requirements.
Many of the evaluations I provide — particularly forensic and immigration evaluations — are not eligible for insurance reimbursement because they are not considered medically necessary services. For clinical evaluations, operating outside of the insurance model ensures that assessments can be fully tailored to your specific needs, without limitations on the types of services provided, the depth of evaluation, or the selection of assessment tools.
Preparing insurance reimbursement documentation typically requires assigning a medical diagnosis and procedure codes that may not reflect the true purpose or scope of the evaluation. To preserve the integrity and accuracy of the services provided, I do not generate documentation for insurance billing or reimbursement purposes.
Payment is due directly from the client prior to the release of the final report. This approach supports thorough, objective assessments designed to best serve the needs of each client and referring professional.
Immigration Evaluations
Immigration-related psychological evaluations are offered for a variety of immigration proceedings, including hardship waivers, asylum cases, T-visa applications, and other immigration relief processes.
In most cases, the type of evaluation needed is already determined based on the client’s legal situation. An initial meeting is still required to gather background information, review referral questions, and discuss procedures and expectations for the evaluation process.
Immigration evaluations are billed at a flat rate, which includes the initial meeting, clinical interview(s), administration of any necessary psychological measures, and preparation of the written report. Additional fees may apply for expedited services or preparation of an affidavit or declaration.
I do offer a limited number of pro-bono and low-bono immigration evaluations. Please contact me directly to discuss your unique circumstances.
Forensic Evaluations
For forensic evaluations, a signed service agreement is required before services are rendered. Attorneys or agencies are responsible for arranging payment of the initial retainer, which is applied toward the evaluation costs. Please contact Van Horn Forensic & Psychological Services directly to discuss case details, estimated fees, and to receive a proposed service agreement. Forensic evaluations are billed either at a flat rate or on an hourly basis, depending on the nature of the referral and scope of services required. Flat rates are available for common types of evaluations including competency to proceed evaluations and guardianship evaluations.
